Blue Courtyard Tile Purchase Order Form

These are the blue tiles that form a pattern in the center of the museum courtyard. Below are form blocks for up to three tiles. Note that blank spaces between words are part of the 20 spaces available per line (2 lines max.) and print in upper case block letters. Do not use periods, commas, semicolons, or anything other than letters or numbers.

The cost for each tile is $50 for one line or $75 for two lines. Blank tiles surrounding a tile or group of tiles are $25 each.  Please enter in the box below the number of tiles you are purchasing and the total dollar amount.  (Also, if you would like the Tile Certificate that we normally send to the purchaser to be sent to the person receiving the gift, please enter the name and address for the gift certificate. Add any text if desired such as "This is from the Jones family" or "Happy Birthday, Dad", etc.. that we would include with the certificate.)
